Medical plans

What is the Healthy Savings Plan?

Lower contributions, a higher deductible, and access to an HSA.

Healthy Savings Plan Basics

  1. Preventive care is covered 100%, no cost to you.
  2. One deductible and OOP maximum shared by you and your dependents.
  3. Once the deductible is met, you pay a coinsurance until meeting the OOP max.
  4. Great for individuals and families that don't expect many medical costs.

Open Enrollment: choose and use coverage

  1. 1

Compare both medical plans and your own enrollment rates.

  2. 2

In Oracle, open Benefits and select Make Changes.

  3. 3

Verify dependents. One BCBS card covers medical and prescriptions; verified dependents receive their own cards.

  4. 4

After your card arrives, register on the BCBS portal to find providers and view claims.

How are my prescriptions covered?

Check your medication, your plan’s cost share, and any approval.

More about this benefit

Your medical plan determines your share

Prescription coverage is included with your medical plan. Costs depend on the medical option and whether the drug is generic, formulary brand or non-formulary brand.

A cost that does not count toward the maximum

Copayments for the difference between a brand-name product and its generic equivalent do not count toward the out-of-pocket maximum.

Check before you fill

  1. 1

Review covered medications in the MyPrime formulary.

  2. 2

Compare local pharmacy prices, generic options and home delivery.

  3. 3

Ask your physician to complete any required prior authorization, step therapy or quantity-limit form.

What the approval terms mean

  1. 1

Prior authorization: certain medications need approval.

  2. 2

Step therapy: another clinically appropriate or lower-cost medication may need to be tried first.

  3. 3

Quantity limit: a dose outside recommendations may require additional explanation.

Next step

Use MyPrime to check the formulary, compare prices or find approval forms.

More about this benefit

What is included

Medical and prescription coverage share one deductible and out-of-pocket maximum. The plan uses the Blue Cross Blue Shield network. Covered in-network preventive care is paid at 100%. An HSA and LPFSA are available, subject to their eligibility rules.

Who can be covered

You and eligible dependents, including eligible children up to age 26. Dependent documentation must be received and approved before coverage is sent to the carriers.

What the ceiling does not include

Your contributions, balance-billed charges and care the plan does not cover are not included in the out-of-pocket maximum. Out-of-network care may cost more.

Next step

Compare the deductible and weekly contribution before choosing your medical plan.
